Note: this survey is concerning lack of medical care for financial reasons only.  This includes inability to afford, access transportation, or for other financial reasons lack the desire or availability for examination and/or treatment.  Do not mark options for people who avoid care for fear of medical examination or treatment.

1. Have you, or have you known someone who may have, chosen sickness and/or death over trying to get well due to financial concerns?
2. Are you, or do you know someone who is, avoiding necessary attention to a medical problem because finances are not adequate or needed elsewhere?
